 Forms and Formats

All rules must be prepared in a standard format. The Secretary of State rule, CSR 153-6, "Standard Size and Format for Rules and Procedures for Publication of the State Register or Parts of the State Register," describes the formatting requirements.

All forms are provided on-line in portable document format (pdf) for use with the Acrobat Reader. Forms are activated for completing on screen -- just position the mouse over the blank area on the form. When the hand icon changes from a hand to an cursor bar, click to place you cursor. Type the information in all the boxes, then print the completed form for filing.

To download Acrobat Reader or see tips for using it, go to Acrobat Reader Help.
